ACP provides support to consumers through four Programs:
Programs:
- Access and Engagement: intake and short-term care
- Community Care: ACP's term for Case Management
- Counseling and Therapy: individual, group and family care in schools, office and home settings
- Service Coordination: facilitating engagement between providers and families/client systems to solve complex needs
Position Overview:
Anazao Community Partners (ACP) is seeking a dynamic and mission-driven clinician to join our Access & Engagement Team, the essential front door to our agency’s behavioral health services. This role is more than intake—it’s where trust is built, hope is restored, and the clinical roadmap begins. The Access Therapist plays a critical role in shaping a client’s first experience of care and guiding them toward the most appropriate treatment pathways.
The ideal candidate is clinically skilled, system-savvy, and confident in diagnostic and decision-making processes. This role requires the ability to quickly assess client needs, clearly communicate service options, and work collaboratively across the agency to ensure timely and effective care delivery.
Key Responsibilities
1. Comprehensive Assessment & Diagnosis
- Conduct thorough but focused biopsychosocial assessments, using a mix of clinical screening tools (i.e. PHQ-9, DLA-20, GAD-7, DAST, etc.) and our EHR diagnostic assessment tool.
- Prioritize diagnostic clarity, especially in identifying substance use, SPMI, trauma, and co-occurring conditions.
- Use clinical judgment to establish accurate diagnoses that lead to appropriate treatment planning.
2. Client Engagement & Orientation
- Explain the purpose and process of assessment in accessible and engaging language.
- Communicate the value of our services, setting clear expectations about what treatment entails (frequency, cost, duration), applying our Levels of Care to client situations.
- Foster trust and hope during the first point of contact.
3. Navigation & Decision-Making
- Serve as a system navigator, connecting clients to internal or external services based on clinical fit, acuity, and preferences.
- Make timely, confident referrals when Anazao is not the appropriate provider.
- Evaluate and document risk with clarity and clinical soundness.
4. Timeliness & Documentation
- Complete assessments and documentation within agency timelines, generally 72 hours.
- Coordinate with Access and Engagement team and with receiving clinicians to prevent service delays.
- Maintain workflow discipline that supports access to care and system-wide efficiency.
5. System Collaboration & Continuity
- Demonstrate fluency in Anazao’s range of services and pathways of care.
- Ensure smooth clinical handoffs through effective communication of key client data and needs.
- Participate in continuous quality improvement related to Access and Engagement.
6. Professional Presence
- Project clinical confidence and composure in a fast-paced environment.
- Instill a sense of direction and hope for clients, even amid complex or acute presentations.

Qualifications:
Educational Background
- Bachelor’s or Master’s (Preferred) degree in Social Work, Counseling, Psychology, or a related field.
Licensure
- Active licensure in the State of Ohio (LSW, LPC, MFT or LISW, LPCC, IMFT (independent preferred) or equivalent.
Experience
- Demonstrated experience with diagnostic assessment and engagement of diverse populations, including youth and adults.
- Competence in both mental health and substance use symptomology and treatment matching.
- Strong written and verbal communication skills.
- Ability to work independently while contributing to a multidisciplinary team.
- Familiarity with Medicaid documentation and evidence-based treatment approaches preferred.
