Hazelwood is an Intermediate Care Facility located in Louisville, KY operated by New Vista. We are a non-profit, mission driven organization dedicated to providing high quality compassionate care to individuals with intellectual and developmental disabilities. Along with providing our residential dental services, Hazelwood also extends it's dental clinic for services to Louisville's community-based IDD residents.
Summary/Objective
The Dental Department serves as the primary dental care for the residents of Hazelwood to include acute and chronic dentistry services. Typical services provided but not limited to are x-rays, cleanings, and restorations. All services are provided at the highest standard of professional practice, complying with standards by both federal and state regulations and facility policies.
Required Education and Experience

Completion of a doctorate program in dentistry

Must have current, active, license from Kentucky State Board of Dentistry
Position Type/Expected Hours of Work

Part-time: 3 days (24 hours per week). Typical Hours are 8:00 am to 4:30 pm with some room for flexibility
Essential Job Duties

Provide comprehensive diagnostics and treatment services for each client.

Participate in developmental reviews and update in individual plans either in person or written report to the Interdisciplinary Team.

Complete intraoral and outer oral exams using all diagnostic aids necessary to properly evaluate the client’s condition no later than one month after admission to the facility {unless examination was completed within 12 months prior to admission}.

Diagnostics and examinations performed at least annually, including radiographs when indicated and manifestation of system diseases.

Reviews results of examination and enters results in the client’s electronic record.

Availability for emergency dental treatment on a 24-hour basis or appropriate arranged coverage for emergency dental services.

Continues professional development, and keeps pace with current job specific literature, technological advances, policies, laws, etc.

Completes all required training activities for position.

Maintains licensure and/or certification required for position.

Shall meet the requirements of 902KR2410 Specialty Intermediate Care Clinic as warranted.
Supervisory Responsibility
This position has supervisory responsibility over the Dental Assistant and Dental Hygienist.
