We are seeking to hire a Social Services Assistant.
Social Services Assistant Qualifications:
- High School diploma or equivalent.
- Social Services designee or eldercare experience required.
- Skilled Nursing Facility experience preferred.
- Point Click Care (PCC) experience preferred.
Looking for a Social Services Assistant who:
- Is mission driven with a passion for working with elders.
- Can schedule and prepare for care conferences.
- Checks MDS schedules and completes interviews with residents.
- Enjoys meeting with new residents and completing assessments.
- Prepares referrals for future needs of the residents.
