Support physicians and healthcare staff by providing updates on the status of patient transfers and locations, stay informed about patient changes, and facilitate efficient communication among interdisciplinary teams.

Support physicians and healthcare staff remotely by providing timely and accurate updates on patient transfers. Communicate patient changes to transfer RNs and interdisciplinary teams.

Maintain accurate and relevant patient documentation and log through virtual platforms. Document transfer encounters and communications. Input details into the referral tracking system with efficiency and timeliness.

Work independently on time management, prioritization, and overall support to ensure the transfer center operates efficiently.

Identify opportunities for process improvement and suggest enhancements to streamline transfer procedures.

High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
